At Transitions Housing Solutions CIC, we have built a deep understanding and expertise in housing combined with support services, emphasising a person-centred and trauma-informed approach.
Our team is dedicated to delivering housing and support solutions for vulnerable populations, including those who have encountered homelessness due to the ongoing difficulties in the housing market or those recognised as having specific housing and support needs.
We are currently able to provide a range of accommodation options including:
· Shared housing for single adults in small fully furnished HMO style accommodation.
· Self contained flats for single adults and couples.
· A limited number of family homes for families in need of support
We are able to offer support to:
Male and females adults (18+) and families, who have been homeless or are at risk of homelessness and one or more of the following:
· complex support needs
· Alcohol or drug related issues
· Ex offenders, involved in the Criminal Justice System
· Care leavers
· Survivors of domestic abuse
· Street homelessness
· Physical or mental health Issues
· Recently released from hospital or rehabilitation facilities and various other issues
We offer quality housing options, including shared accommodations, typically in HMO style properties located in and around Nottingham, accommodating a maximum of four service users
Our housing solutions include smaller properties designed for individuals with lower support needs, particularly those assessed as lower risk or who have previously exhibited behaviours that may hinder their ability to live in shared environments.
After receiving a referral, we evaluate the individual's support requirements through a phone call or an in-person meeting. In this conversation, we explore the support that the person believes will help them transition to independent living.
We develop support plans and carefully document every action and progress made towards achieving the goals of these plans, helping individuals transition from supported accommodation to their own independent living in either the private or public housing market.
Our commitment is to support service users in enrolling in training, education, or job opportunities, which can provide them with structure and valuable skills such as:
Key Skill in Maths and English
Time keeping and organisation
Interpersonal communication
Accountability
Functional Skills
Vocational Skills
We collaborate with educational institutions that provide courses in GCSE English and Maths, Hair and Beauty, Motor Mechanics, Engineering, Electrical Installation, and Building Skills.
We also manage properties for landlords who feel overwhelmed or simply lack the time to handle their properties and tenants, especially when dealing with the complexities of the benefits system and the pursuit of rent payments.
We have in-depth knowledge in helping those who rely on housing benefits and universal credit, and we support residents in handling their rent accounts on your behalf. Additionally, we can manage your property long-term, ensuring it is well-kept and leased, so you can receive consistent income without the burden of rent collection and repair arrangements.
